Everyone knows we care for the elderly in their home, usually during the day, but what you may not know is that a Blossom carer can also come to your home to stay overnight and either stay awake to provide whatever assistance or monitoring is necessary, or simply sleep in your home overnight to provide peace of mind. Night Care is often used to provide a break for the full-time carer on a short-term basis, or over longer periods, as required.

Travelling away from home without a companion can be especially daunting for the elderly or disabled so if you need company to travel away for a holiday or medical care at Blossom, we offer Care Companions to travel with you. Staying in the same accommodation, or close by, they are available whenever you require assistance or company. As a holiday companion, the Carer can also share the experiences with you, providing engaging company so that you never feel alone whilst travelling.

At Blossom we also provide complex care services for those living with a long-term condition. Often people with these conditions want to maintain their independence and stay in their home for as long as possible, but fear that as their condition progresses, they’ll have to move to a nursing or residential home. At Blossom our aim is to enable you to remain in your own home for as long as possible, so we deliver quality care, overseen by a nurse and tailored to suit your complex care requirements.

If you have an illness that can’t be cured, palliative care is offered to make you as comfortable as possible, by managing your pain and other distressing symptoms. You can also receive palliative care earlier in your illness, while you are still receiving other therapies to treat your condition. Palliative care also involves the psychological, social and spiritual support of you and your family or carers. It’s a holistic approach that focuses on the “whole” person recognising the right to express personal wishes about where you would like to receive care and, when appropriate, where you want to die.

Whichever high quality care management service we provide, our process commences with an assessment of needs so we can develop a detailed person-centred care plan that identifies all of your requirements and implements a highly effective care service for you. We regularly review this document with you to ensure that we continuously adapt our care service to meet your changing needs and preferences.
