The commencing of end-of-life care is an intensely personal experience. When this difficult time comes, every person now has a choice of where they choose to end their days.


You no longer have to die in hospital; you can request to move home so you can pass away in the familiar and comforting surroundings of your own home, with family and friends by your side, should that be your request.


Whatever your choices are and however long palliative care may last, we treat everyone with the utmost dignity and respect. Blossom Home Care carers are expertly trained in the delivery and management of compassionate end-of-life and palliative care in your own home. Indeed, end-of-life care should help patients live as well as possible until they pass away – something we enable whilst supporting family and friends however we can.


Palliative care can last for several months depending on the client’s condition. Therefore every palliative care service we provide is completely tailored to the individual, respecting all their needs and wishes.


As well as constant cooperation with the client and their family, our carers also work very closely with local NHS healthcare providers and specialist nurses. This ensures the holistic care they are delivering is perfectly suited to the situation at all times.

What makes Blossom Home Care’s palliative care different?


We go above and beyond to make this incredibly tough time as easy to manage as possible for the client and their family. This includes:


A fully tailored enhanced palliative care service

Exactly what care clients want and need is incredibly specific to them. From the outset we work with you to understand every last detail, so the client always receives exactly the right care and treatment at all times.


During this stage of their illness, clients are often unconscious for long periods of time. Therefore we carry out end-of-life wishes via loved ones or by using pre-written plans.


Working with multidisciplinary teams (MDT)

Our highly trained and experienced carers work extremely closely with the client’s family and multidisciplinary team (MDT) from the first moment we are enlisted to help, including:

● GPs

● District nurses

● Specialist nurses, such as those from Macmillan Cancer Support and Marie Curie


This partnership-first approach allows us to carry out a precise, fully tailored enhanced palliative care plan that achieves the right outcomes for the patient and keeps them comfortable.


Complete respect of people’s religious beliefs and traditions

Our care teams have total empathy and understanding of clients’ personal religious beliefs and traditions. When planning your end-of-life care, you will be asked about these wishes and any preferences in other areas – such as requested music to be played – so that these can be taken into account.


Award-winning palliative care

We are proud to have won multiple awards for our palliative care. Most recently this saw us receive a Commendation in Palliative Care Team Award at the Great British Care Awards.


Putting empathy at the forefront

For us, palliative care is all about empathy. This means providing emotional support for everyone involved during these very difficult times, including family, friends, and anyone else important to you, as well as the wider support team and carers themselves.
