Our online feedback system means everything is recorded including carer rota’s, visits and updates from the carers about the outcomes of their visit to a client. As well as working directly with service users we also receive referrals from/work in partnership with both the private and public sectors throughout the North East and Yorkshire, including Case Managers, NHS Clinical Commissioning Groups, Social Services and Local Authorities, Solicitors and Insurance Companies.

Case Managers

Sourcing a care team can be a costly and time consuming business so it’s important to get it right first time. So, when your client’s present clients with these conditions you can depend on Blossom Home Care to deliver the care element of your case, leaving you free to spend more time on your client’s rehabilitation. Our specialist, evidence based, care and support packages for Adults who have sustained injuries as a result of accidents or medical negligence are second to none. And, because we also know that you need to be able to obtain accurate information quickly, in a format that can be easily incorporated into reports and recommendations made on behalf of your clients we also provide that detail too. Our online feedback system records carer rota’s and updates from the carers about the outcomes of the client visit. NHS Clinical Commissioning Groups (PCTs), Social Services and Local Authorities

Some of the clients we work with access Continuing Healthcare Funding to meet the cost of their care. Administered regionally the Clinical Commissioning Groups (CCGs) access various NHS funds to deliver homecare for complex healthcare needs such as those deriving from spinal injury conditions, acquired brain injuries and other neurological illnesses or conditions.

Working closely with CCGs and the families of service users Blossom’s process begins with an assessment so that when we appointment your Blossom Home Care Team we know they are able to fully meet the individual client needs. We then set about delivering the agreed care plan, continuing to work with the CCGs and the families of service users to ensure the desired quality and care outcomes are achieved/maintained. Whether privately funded, case management settlements or PHB’s (Personal Health Budgets) the flexibility to offer our service user’s additional short-term or long-term support to accommodate their changing medical needs is also important.

Personal Health Budgets (PHBs)

If you, or a family member, suffer with a long-term condition or disability, you may qualify for a Personal Health Budget. NHS funding via a PHB is designed to give you, the individual with the condition, greater choice and control over your health and wellbeing support. Council funding for Healthcare and Disability Homecare Support

Some individuals access Social Services funding administered by their local authority in a very similar process to the one followed to obtain a PHBs. Blossom Home Care also work with Social Services Case Managers and the families of service users to tailor care plans for each. Solicitors and Insurance Companies

Some of our clients are pursuing Personal Injury or Medical Negligence claims through Solicitors or Insurance companies. The complex Health and Rehabilitation issues associated with dealing with Personal Injuries while, at the same time starting, and then continuing with, a litigation process makes the needs of these clients different from our other clients.

For this reason we have specifically developed our services to meet these needs and also to meet the requirements of Case Managers by providing our specialist evidence based care and support packages to Adults, who have sustained injuries as a result of accidents or medical negligence.
